Forge Medical Practice are working with the Sunderland Armed Forces Network to ensure that the above are recognised and are managed appropriately within the NHS under the Armed Forces Covenant.

Please contact the surgery if you have any queries in regard to identifying yourself to the practice as a veteran. The information obtained will be kept confidential.
